About
Ovok is a FHIR R4 backend for building digital health apps, with React and React Native SDKs. It stores data from 50+ supported devices such as blood pressure monitors, sleep radars, and SpO2 monitors in FHIR. Teams choose which patients are enrolled in Signals, and only eligible vital-sign readings from those patients are sent for alert evaluation. The page states Signals is MDR Class IIa and that Ovok itself is not a certified medical-device product.
At a glance
Ovok, an Actimi product, is a hosted FHIR R4 backend for teams building remote patient monitoring apps. Readings from supported devices are stored as FHIR records, and React and React Native SDKs build the patient and clinician apps on top. An optional component, Signals, evaluates eligible vital-sign readings for patients you enroll and returns alert decisions. Ovok states that Signals is MDR Class IIa and that Ovok itself is not a certified medical-device product.
Good fit
A digital-health product team that wants to ship a monitoring app without building its own FHIR server, device ingestion, identity and audit layer. It suits teams already working in React or React Native who want EU hosting: every published plan lists AWS Frankfurt. Ovok publishes implementation guides for blood pressure monitoring, CHF monitoring, type 2 diabetes screening and medication reminders.
Consider something else when
You want a finished clinical product. Your team builds the app and its care workflows, and Ovok says a product built on it does not inherit Signals' regulatory status. Also skip it if you need billing or RCM APIs (listed as roadmap with no date), a ready-made lab connector, or a hosting region other than AWS Frankfurt, the only one in published pricing.
Start here
Choose "Start free" for the Developer sandbox: one environment, 50 test monitored patients and a hard cap of 10,000 API requests a month, with test data only. Check your project's CapabilityStatement for the enabled FHIR interactions, then follow the Blood Pressure RPM guide, where Bluetooth readings are saved as FHIR Observations for the app to read back. A physical device can be tested through the Native SDK; live ingestion through the Ovok IoT Service needs setup with the Ovok team.
Cost and requirements
As of 1 November 2026, prices are in EUR excluding VAT. Developer is free. Growth costs €2,000 a month on annual billing for 250 monitored patients, then €7 per extra patient; Scale costs €6,000 for 1,000, then €5. Monthly billing is 20% higher. Enterprise is quoted, and a dedicated stack starts at €29,500 a month. A monitored patient is one with at least one Signals evaluation that month; a second device costs €2 a month. Paid plans include a GDPR Art. 28 data processing agreement.
Limits to know
Ovok's documentation says a missing Access Policy is not restrictive, so you must define the rules your project needs. Device telemetry retention is 7 days on Developer and 24 months on Growth and Scale, and it does not cover FHIR Observations or other clinical records, whose retention Ovok says needs separate confirmation. Ovok also says a match in its 50+ device catalogue does not mean every model, firmware version or deployment path is supported. Scheduling, imaging and CRM need app-specific integrations.
Still unclear
- What intended purpose does Signals' MDR Class IIa status cover, and does it fit your alerting use case? The captured pages state the class but not the scope.
- What retention terms apply to FHIR Observations and other clinical records on each plan?
