Polygraph aims to preserve the of a software change across repositories and developers. In Victor Savkin's presentation, it combines a map of repository dependencies with recorded conversations, code revisions and pull requests. That gives a later session access to both the affected code and the discussion that produced it.
The same records support handoffs: another developer can reconstruct the captured repositories and session history on their machine.
