Retries and idempotency

Recovering from failure without duplicating a completed side effect.

A retry repeats work after a transient failure; an idempotent operation makes repetition safe. Agent tools need stable operation IDs, recorded outcomes, bounded retry policies, and clear distinctions between unknown and failed state.
